Care Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Collaborate with members and their families to determine supports and direct services related to health, employment, and goals for independent or community living.

• Work alongside clinical and social support colleagues to meet the physical, behavioral health, and long-term service needs of members.

• Perform detailed, objective face-to-face visits in home or community settings.

• Evaluate members’ needs in the areas of physical, behavioral, functional, psychosocial, financial, employment, and independent living.

• Utilize established criteria to authorize suitable home- and community-based services.

• Ensure that the authorized services are provided and that members’ needs are adequately addressed.

• Travel to the residences of members for visits.

• Handle requests from both internal and external stakeholders.

⛳️ Requirements

• A minimum of 3 years of experience in a clinical environment.

• At least 2 years of clinical experience is mandatory.

• Active registered nurse license in Tennessee, or an active license in the applicant’s state of residence if partaking in the Nurse Licensure Compact Act.

• Alternatively, a Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license is acceptable.

• Outstanding customer service skills are essential.

• Residency in McMinn County, Tennessee is required.

• Must be available for an 8:00am–5:00pm EST schedule.

• A valid driver’s license is necessary.

• TB Skin Test is applicable to coordinators working in the field.

• Proficiency in basic Microsoft Office and email is needed.

• Strong time management skills are a must.

• Excellent oral and written communication abilities are required.

• Strong interpersonal and organizational skills are essential.

• Must maintain auto insurance compliance and an acceptable motor vehicle record while driving for company-related activities.

• Completion of a digital literacy assessment is required.

• Various immunizations and/or related medical tests may be necessary.
