Care Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Collaborate with members and their families to identify necessary supports and direct services for health, employment, and goals related to independent or community living.

• Work alongside clinical and social support teams to address members’ needs in physical health, behavioral health, and long-term services.

• Perform comprehensive, objective face-to-face assessments to evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living requirements.

• Utilize established criteria to authorize suitable home- and community-based services.

• Verify that authorized services are being delivered and that members’ needs are being fulfilled.

• Travel to the homes of members for assessments and visits.

• Handle requests and demands from both internal and external stakeholders.

⛳️ Requirements

• Minimum of 3 years of experience in a clinical environment.

• At least 2 years of required clinical experience.

• Active registered nurse license in Tennessee, or an active license in the state of residence if participating in the Nurse Licensure Compact Act.

• Alternatively, a Master’s in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).

• Outstanding customer service abilities.

• Must reside in Wayne or Lawrence County, Tennessee.

• Availability for an 8:00am–5:00pm EST work schedule.

• Possession of a valid driver’s license.

• TB Skin Test is required for coordinators working in the field.

• Proficient in basic Microsoft Office applications and email.

• Strong time management skills.

• Excellent verbal and written communication capabilities.

• Solid interpersonal and organizational skills.

• Completion of a digital literacy assessment is mandatory.

• Various immunizations and/or relevant medical tests may be necessary.

• Employees who regularly operate a company, personal, or rental vehicle must participate in the BCBST Driver Safety Program, maintain a valid driver’s license, adhere to auto insurance requirements, and uphold an acceptable motor vehicle record.
