This is a fully remote position, open to applicants in Illinois.
• Evaluate the care needs of members and ensure that services are delivered in the least restrictive environment.
• Identify and address obstacles to effective care.
• Develop and execute interventions to satisfy care requirements.
• Coordinate services and assess case management plans against the individual goals of members.
• Continuously track patient care and progress towards desired outcomes through assessments, data analysis, discussions, and proactive care planning.
• Conduct assessments and evaluations via telephone and in-person.
• Address immediate and critical gaps in care and access.
• Support enrollees in filing grievances and appeals.
• Clarify MCO requirements, services, and benefits.
• Offer information and referrals to community resources.
• Assist members and their families in navigating suitable care resources.
• Facilitate communication with payers, providers, interdisciplinary teams, and others involved in the care of members.
• Travel to hospitals as required.
• Bachelor's degree in social sciences, social work, human services, or a related discipline; OR an unrestricted Licensed Practical Nurse (LPN) in Illinois with one year of experience performing comprehensive assessments and delivering formal services to the elderly; OR Licensed Professional Counselor (LPC); OR Licensed Social Worker (LSW); OR Qualified Mental Health Professional (QMHP).
• Must be a resident of Illinois.
• Minimum of 1 year of experience in healthcare and/or case management.
• At least 1 year of experience with Medicare and Medicaid recipients, long-term care, Home and Community Based Services, and/or managed care organizations.
• Proficient in using electronic medical records and various electronic information applications/software programs.
• Valid state driver's license required.
• Personal vehicle liability insurance that meets or exceeds the stated minimum limits.
• Availability to work Monday–Friday during operational hours of 8:00 AM–7:00 PM CST, primarily from 8:00 AM–5:00 PM CST, with flexibility in scheduling.
• Capability to fulfill remote work internet requirements of a minimum of 25 Mbps download and 10 Mbps upload.
• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information.
• Required TB screening upon selection.
• Willingness to travel up to 25% to visit members in hospitals within a 50-mile radius.
