Care Advisor – Chronic Care Management

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform regular follow-up calls with the designated patient group via phone.

• Create and uphold detailed care plans.

• Facilitate coordination of care among healthcare professionals and services.

• Record all patient interactions within the EHR system.

• Monitor and assess patient progress and adherence to treatment.

• Recognize and resolve obstacles to care.

• Educate patients on managing chronic health conditions.

• Achieve monthly performance metrics and quality objectives.

⛳️ Requirements

• Medical Assistant certification is highly preferred; LPN and RN qualifications are also accepted.

• At least 3 years of clinical experience in a healthcare setting or medical office.

• In-depth understanding of chronic conditions and care management practices.

• Proficient in Electronic Health Records (EHR) systems and acquainted with clinical workflows.

• Outstanding communication and interpersonal abilities.

• Background in patient education and care coordination.

• Strong analytical skills and capacity to work independently.
