Bilingual Telephonic Care Coach

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Assess the needs and requirements of members to achieve or maintain optimal wellness.

• Guide members and their families towards suitable care and wellness resources.

• Coordinate services and resources to enhance members' wellness.

• Identify and address obstacles to effective care.

• Monitor patient care and track progress towards desired outcomes through assessments, data analysis, conversations, and active care planning.

• Facilitate engagement with healthcare and community resources.

• Make decisions regarding work methods and priorities while adhering to established procedures, regulations, and professional standards.

⛳️ Requirements

• Bilingual in English and either Spanish, Polish, Arabic, Creole, or another language.

• Bachelor's degree in a health or human services field OR be an LPN with the capability to obtain licensure in Illinois.

• A minimum of 2 years of experience in the healthcare field.

• Proficient in Microsoft Office Word and Excel.

• Outstanding verbal, written communication, and interpersonal abilities.

• Competency in using various electronic information applications and software programs.

• A strong desire to contribute to an organization dedicated to continuously enhancing consumer experiences.

• A separate room with a locked door suitable for a home office to guarantee ongoing privacy.

• Minimum internet speed of 25 Mbps for downloads and 10 Mbps for uploads at home.

• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information.
