Bilingual Care Coordinator – Spanish, Vietnamese

This is a fully remote position, open to applicants in California.

📋 Description

• Engage in outreach activities to assist members with referrals, authorizations, medication refills, provider appointments, and other healthcare needs.

• Execute tasks and assessments within the Case Management system for discharges from hospitals and skilled nursing facilities (SNFs).

• Work collaboratively with RN Case Managers to oversee a group of SNP members.

• Keep track of alerts regarding changes in members' conditions, admissions, discharges, and diagnoses, and relay updates as necessary.

• Establish trust with members by serving as a liaison and advocate for their healthcare.

• Record interactions and ensure accurate maintenance of member records.

• Facilitate communication with providers, which includes faxing or mailing documents and requesting medical records.

• Participate in interdisciplinary team meetings and engage in member case reviews.

• Uphold confidentiality and adhere to all departmental deadlines.

⛳️ Requirements

• Bilingual proficiency in Spanish or Vietnamese (mandatory).

• At least 1 year of experience in a healthcare Independent Practice Association (IPA) setting.

• A minimum of 1 year of experience handling referrals and prior authorizations.

• High school diploma or GED is required.

• Basic computer proficiency (Microsoft Outlook, Word, Excel).

• Typing speed of 25+ words per minute.

• Excellent verbal and written communication abilities.

• Strong organizational and time management skills with the capacity to prioritize tasks in a fast-paced environment.

• Familiarity with Medi-Cal and managed care plans.

• Certification as a Medical Assistant or in Medical Terminology (preferred).

• Previous training in medical terminology or clinical support roles (preferred).

• Ability to sit, talk, type, and use hands frequently.

• Occasionally required to lift up to 10 pounds.
