Bilingual Patient Coordinator, Spanish – English

This is a fully remote position, open to applicants in Peru.

📋 Description

• Act as a connection point among contracts, patients, their families, Home Health Aides (HHAs), and other individuals involved with the clients’ care team.

• Facilitate effective communication and pinpoint areas of concern during attendance verifications, introductory calls, and various interactions.

• Execute necessary tasks in accordance with agency protocols and policies.

• Organize work assignments based on the training and experience of Home Care Workers.

• Reduce the number of open cases to ensure prompt resolutions.

• Accurately document all communications.

• Engage with patients, family members, contractors, clinicians, aides, and other departments concerning issues or modifications.

• Supply essential information to other healthcare professionals.

• Collaborate with family members and other support systems to guarantee continuous care and address client needs.

• Inform HHAs about discharges, alterations in service days/hours, and other pertinent updates.

• Prepare daily, weekly, and monthly reports.

• Oversee requests for overtime and travel time.

• Utilize tools to track aides, provide directions, and verify availability.

• Fill out incident reports.

• Manage mutual and linked cases.

• Promptly open and respond to emails, voicemails, and faxes.

• Adjust to changes within the agency or industry.

• Address concerns or inquiries with the supervisor.
