Bilingual Nurse Practitioner

This is a fully remote position, open to applicants in California.

📋 Description

• Provide treatment, management, and guidance to individuals pursuing diabetic reversal and enhancing overall health across various conditions.

• Assist members across multiple states through remote care management.

• Collaborate with health coaches, physicians, the chief medical officer, and other healthcare professionals.

• Conduct comprehensive health assessments, including laboratory results, medical history, and psychosocial evaluations.

• Evaluate symptoms and administer treatment as necessary, working in conjunction with members' primary care providers.

• Deliver care via telemedicine and document interactions in Twin’s clinical platform.

• Manage a high-risk population while addressing social determinants of health and specific needs of the population.

• Acquire knowledge of and implement the Twin Model of Care for diabetes and chronic condition reversal.

• Partner with the medical team to ensure exceptional customer service and member experience.

• Provide patient education to encourage disease prevention and promote overall health.

• Review medical histories, symptoms, allergies, and medications diligently.

• Pose situation-specific inquiries to accurately assess symptoms.

• Prescribe medications and deliver dosage and administration instructions in accordance with Twin Health policy.

• Maintain precise patient records, including contact information, medical history, medications, allergies, diagnoses, and progress notes.

• Work Monday to Friday, with three shifts from 11 am to 8 pm and two shifts from 9 am to 6 pm PST.

• Perform additional tasks as assigned.

⛳️ Requirements

• ARPN CA state license, ideally licensed in multiple states.

• Proficient in both English and Spanish.

• 3-5 years of experience as an advanced practice provider.

• Clinical experience with diverse populations, including underserved communities.

• Experience managing patients remotely across various geographic regions and states.

• Possess 5 or more licenses, with a willingness to obtain additional ones.

• Board-certified in family or internal medicine.

• Proven track record as an advanced practice provider.

• Strong medical knowledge base.

• Capable of consulting with patients through virtual communication channels.

• Familiarity with using technology and data to inform care decisions.

• Excellent analytical and problem-solving abilities.
