This is a fully remote position, open to applicants in United States.
• Conduct telephone consultations with patients participating in care management and/or remote patient monitoring programs.
• Offer support and education regarding patients’ chronic health issues.
• Inform patients about their health conditions and empower them with strategies for lifestyle and behavioral changes.
• Assist patients in setting and achieving goals aligned with provider-approved care plans.
• Keep precise and current patient records and document interactions and care plans according to established protocols.
• Address patient concerns and obstacles to care, finding practical solutions to enhance adherence and outcomes.
• Deliver clear, compassionate, and effective communication to patients.
• Adhere to approved workflows for conveying patient needs to providers.
• Engage in training sessions, team meetings, and initiatives for quality improvement.
• Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, following approved partner workflows.
• Communicate with enrolled patients in collaboration with their care teams.
• Manage a designated group of patients throughout the month and assist with other patients or tasks as assigned.
• Current, valid, and well-regarded Multistate/Compact Nursing License (LPN/LVN).
• Additional state licenses may be necessary.
• Over 3 years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.
• Proficient in verbal and written communication in Spanish.
• Strong communication and interpersonal skills.
• Excellent organizational and time management skills.
• Proficient in using electronic health records (EHR) and care management software.
• Capability to work independently as well as part of a team.
• Empathetic and patient-centered approach to care.
• Reliable internet connectivity.
