Bilingual Care Manager, Telephonic Nurse

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Evaluate and assess the needs and requirements of members in a telephonic setting.

• Direct members and their families towards suitable care and wellness resources.

• Address members' physical, environmental, and psychosocial health concerns.

• Identify and eliminate obstacles to effective care.

• Oversee patient care through assessments and evaluations to ensure progress towards desired outcomes.

• Develop member care plans as necessary.

• Conduct health assessments via phone.

• Offer care planning and education on chronic conditions and gaps in healthcare plans.

• Make independent decisions regarding work methods while adhering to established guidelines and procedures.

• Collaborate with a designated member population.

⛳️ Requirements

• Bi-Lingual (English/Spanish).

• Bachelor's degree.

• 3 - 5 years of clinical acute care experience.

• Licensed Registered Nurse (RN) in the relevant state without any disciplinary action.

• Extensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.

• A strong desire to contribute to an organization dedicated to enhancing consumer experiences.

• Work from a dedicated environment free from ongoing interruptions to safeguard member PHI/HIPAA information.

• Self-provided internet service with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.

• Availability to work between 8:00am-7:00pm Central Standard Time, primarily during core hours of 8:00am-5:00pm CST.

• Occasional travel to Humana offices for training or meetings may be required.
