Bilingual Care Coordinator, Spanish and English Proficiency

This is a fully remote position, open to applicants in Nicaragua.

📋 Description

• Serve as the primary contact for clients, families, caregivers, referral sources, and healthcare professionals by managing a high volume of incoming calls.

• Facilitate care coordination and patient referrals, ensuring timely scheduling of both initial and follow-up appointments.

• Examine doctors’ notes, upload prescriptions, and verify that care plans are updated and accurate.

• Organize, reschedule, and oversee care assignments across various locations utilizing cloud-based scheduling systems.

• Employ scheduling and care management software to log updates, track staff assignments, and oversee caseloads.

• Confirm patient attendance, monitor treatment progress, and follow up with providers to guarantee continuity of care.

• Keep precise client records, document all communications, and adjust care plans as necessary.

• Conduct follow-ups with providers and handle escalations in line with established Standard Operating Procedures (SOPs).

• Engage with field staff, clients, and internal departments to ensure adequate coverage and smooth care operations.

⛳️ Requirements

• Experience in care coordination.

• Preference for applicants with previous experience in home care coordination.

• Ideal candidates should be able to start within one week.

• Familiarity with home care scheduling software and Electronic Health Record (EHR) systems.

• Strong communication skills with the ability to manage a high call volume and build rapport over the phone.

• Knowledge of industry regulations and compliance standards, including HIPAA and state home care regulations.

• Excellent organizational and multitasking skills, with a keen attention to detail.

• Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.

• Openness to learning care management or scheduling systems.

• A compassionate, service-oriented approach, with the ability to maintain composure under pressure.
