This is a fully remote position, open to applicants in United States.
• Deliver outstanding care, disease management, and health education to patients.
• Facilitate individual patient goal setting asynchronously to assist in managing chronic conditions.
• Develop customized action plans aimed at reducing or eliminating undesirable behaviors.
• Aid clients in understanding their motivations and formulating behavior change strategies.
• Perform regular outreach to patients based on their needs and preferences to foster engagement and behavior modification.
• Collaborate with clinical team members to ensure an exceptional patient experience.
• Establish and nurture professional, support-focused relationships with patients and colleagues.
• Create and distribute health education materials tailored to individual members when necessary.
• Work alongside a cross-functional product team to enhance the in-app patient experience.
• Compact state license is mandatory; additional licensing may be required.
• Proficiency in Spanish (speaking, reading, and writing) is essential.
• A minimum of 2 years of experience as a Licensed Practical Nurse or in possession of any nursing license.
• At least 2 years of experience in patient-facing or customer-facing roles.
• Exceptional customer relations, along with strong written and verbal communication skills.
• Familiarity with medical terminology is essential.
• Competence in general medical office procedures is required.
• Experience with digital applications such as Slack, Coda, Google Workspace, and similar tools.
• Strong analytical capabilities and proactive problem-solving skills.
• Self-driven, results-oriented, and a strategic thinker.
• A personal passion for health and wellness topics is preferred.
• Authorization to work in the United States is required.
• Experience in telehealth or a healthcare startup is preferred.
• Background in GI, weight management, and/or anti-obesity medications such as GLP-1s is a plus.
