Behavioral Health Specialist II

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Create and execute personalized crisis and safety plans with youth and their families.

• Facilitate referrals and connections, oversee the implementation of care plans, and manage 1915(c) Waiver funds.

• Secure prior authorization and approval from the Ohio Department of Medicaid.

• Conduct outreach, assessments, meetings, and care coordination either in person, in the community, or virtually based on guardian/member preferences.

• Manage caseloads for Intensive Care Coordination and Moderate Care Coordination.

• Integrate comprehensive, evidence-based strategies into Child and Family Team meetings.

• Engage newly referred youth and families in care coordination efforts.

• Create Child and Family-Centered Care Plans and identify family teams for clients.

• Facilitate monthly or more frequent Child and Family Team meetings.

• Conduct thorough assessments, including Child and Adolescent Needs and Strength assessments.

• Implement trauma-informed care, culturally responsive practices, and the wraparound model.

• Mentor and develop new staff members.

• Coordinate care with providers, agencies, physical health practitioners, and school personnel.

• Monitor team commitments and CFCCP implementation, providing updates to team members on progress.

• Develop community resources to address the needs of youth and families.

• Complete care plan documentation accurately and promptly, billing for care coordination activities.

• Document team attendance, approvals, signed CFCCPs, SMART goals, and authorization processes in the electronic health record.

• Provide consultation on documentation standards and enhancements for the electronic health record.

⛳️ Requirements

• Must be a resident of Scioto or Lawrence County.

• Bachelor’s degree with 2+ years of experience and training in one or more of the following: Family systems; Community systems and resources; Case management; Child and Family counseling or therapy; Child protection; Child development.

• Alternatively, a Master’s degree with 1 year of experience in one or more of the aforementioned areas is acceptable.

• Clinical license, certification, or training may be required.
