This is a fully remote position, open to applicants in Texas.

📋 Description

• Act as the main point of contact for patients participating in the BHI or CoCM program.

• Perform regular behavioral health screenings (e.g., PHQ-9, GAD-7) and monitor patient progress over time.

• Offer patient outreach, psychoeducation, and support in between clinical appointments.

• Collaborate with both the psychiatric consultant and primary care provider (PCP) to assess patient progress and modify care plans as necessary.

• Record all patient interactions, screenings, care plan modifications, and psychiatric recommendations in the medical record.

• Oversee patient adherence to behavioral health treatment plans and aid in follow-up care coordination.

• Ensure that patient consent for collaborative care is obtained and documented before billing.

• Help patients navigate barriers to care (e.g., scheduling, transportation, understanding treatment).

• Manage referrals to specialty mental health providers when needed.

• Support billing operations by ensuring compliance with time tracking, service documentation, and care plan updates as mandated by CMS and commercial insurers.


⛳️ Requirements

• Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, Nursing, Public Health, or a related field.

• Previous experience in behavioral health, care coordination, or chronic disease management is preferred.

• Knowledge of behavioral health screening tools (PHQ-9, GAD-7, etc.) is essential.

• Familiarity with the Collaborative Care Model and/or BHI billing requirements is highly desirable.

• Excellent communication and interpersonal skills to effectively engage with patients, families, and multidisciplinary teams.

• Competence in electronic health record (EHR) documentation and case management processes.

• Bilingual skills are an advantage.
