This is a fully remote position, open to applicants in Kansas.
• Oversee patient flow, triage, and deliver direct patient care.
• Act as a liaison among front desk personnel, nursing staff, physicians, and the clinic manager.
• Manage patient populations by establishing expected outcomes, defining resource utilization, and collaborating with providers to set patient goals.
• Offer nursing support and coordination through various clinical, educational, and supportive programs.
• Encourage patients to manage their own disease effectively.
• Ensure continuity of care following hospital admissions, discharges, and emergency room visits.
• Oversee high-risk patients, including those with multiple comorbidities or a significant risk of readmission, utilizing a registry system.
• Evaluate patients' physical, psychosocial, financial, and environmental healthcare needs.
• Provide education to patients and ancillary staff regarding healthcare needs, safety concerns, medications, diagnostic procedures, nutrition, and overall wellness.
• Utilize EMR and electronic tools to document assessments, treatment plans, and patient progress.
• Monitor, document, and report on patient conditions and their responses to medications and treatments.
• Maintain and review patient records, charts, and test results; follow up with patients as necessary.
• Address and redirect treatment-related phone inquiries while offering medication and treatment instructions.
• Uphold patient confidentiality at all times.
• Assist new cardiology patients by identifying suitable providers, gathering history and medical records, preparing charts, and scheduling appointments.
• Support designated physicians during clinic sessions and relay patient information and care plans efficiently.
• High School Diploma.
• Bachelor’s Degree in Nursing.
• Minimum of 2 years of Registered Nurse (RN) experience.
• Current, active multistate Registered Nurse (RN) license in good standing in your residing state.
• Basic Cardiac Life Support (BLS or BCLS) certification from the American Heart Association to be obtained within 14 days.
• Proficient typing and word processing skills.
• Strong analytical, critical thinking, and problem-solving abilities.
• Prior experience in a medical office setting is preferred.
