This is a fully remote position, open to applicants in Massachusetts.
• Advocate for rational, safe, and cost-effective medication therapy in partnership with primary care providers, specialists, nurse care managers, community health collaborators, and non-clinical personnel.
• Identify and address medication therapy opportunities while collaboratively designing personalized therapeutic plans for chronic disease management and preventive care.
• Deliver direct care to members and caregivers through Comprehensive Medication Management (CMM).
• Assist hub-based and virtual clinical care teams by providing pharmacist services.
• Create medication management action plans that include measurable goals and monitoring timelines.
• Work together with Cityblock and external providers to ensure interventions and encounters align with medication-related objectives.
• Offer drug information, dosing guidance, interaction management, adverse-effect prevention, and tailored support for members.
• Conduct medication management training sessions and develop educational resources.
• Educate and mentor team members on maintaining accurate medication lists in Commons and performing post-discharge medication reconciliation.
• Establish medication management and disease-state clinical pathways and protocols.
• Aim for collaborative practice agreements that authorize pharmacist assessments, laboratory orders, and medication initiation or modifications according to protocol.
• Possession of an active, unrestricted pharmacist license from the Massachusetts Board of Registration in Pharmacy at the time of hire, which must be kept in good standing throughout the duration of employment.
• Successful completion of both a PGY-1 Pharmacy Practice Residency and a PGY-2 Ambulatory Care Pharmacy Residency, or a minimum of 2 years of experience as a clinical pharmacist in an ambulatory care environment.
• Fulfillment of Cityblock’s medical clearance prerequisites, which include proof of immunity to MMR, Hepatitis B, Varicella, and a TB screening, unless a valid medical or religious accommodation is in place.
