Advanced Practice Clinician, Care Anywhere

This is a fully remote position, open to applicants in California.

📋 Description

• Perform in-home assessments for Alignment members, including medication evaluations and health screenings.

• Educate patients by aligning their desired care with the highest quality of care available.

• Recognize diagnoses for effective care management and ongoing medical oversight.

• Collaborate with the Regional Medical Officer and other team members to formulate suitable care plans.

• Coordinate care efforts with primary care physicians, specialists, and ancillary service providers.

• Lead a diverse clinical team that may encompass nurses, health coaches, social workers, and care coordinators.

• Deliver medical and social assistance to eligible patients of Alignment Healthcare in their residences.

• Assist in preventing unnecessary hospitalizations, health issues, and unmanaged disease progression.

⛳️ Requirements

• Experience: None required.

• Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.

• Currently active Nurse Practitioner or Physician Assistant license.

• Active RN License and Furnishing number required upon hire (for Nurse Practitioners only).

• Valid Nurse Practitioner Board Certification or Physician Assistant Certification.

• NPI Number.

• DEA registration.

• Current BLS certification.

• Valid Driver's License and proof of current automobile insurance.

• Strong ability to communicate positively, professionally, and effectively with others.

• Excellent problem-solving, organizational, and time-management skills.

• Capability to thrive in a fast-paced environment.

• Willingness to conduct home visits in Stanislaus, CA.

• Preferred: One year of previous clinical or home care experience.

• Preferred: Familiarity with Electronic Medical Records (EMR).

• Preferred: Experience in caring for older adult (geriatric) patients.
