Advanced Care Physician

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Assess and approve new hospital admissions or patient transfers into the home care program.

• Perform virtual or in-person rounds to oversee recovery, revise treatment plans, and assess vital signs.

• Work collaboratively with nurses, pharmacists, case managers, and medical specialists to ensure smooth treatment processes.

• Order and analyze lab tests, imaging, and medications specifically designed for a home care environment.

• Oversee safe transitions from the acute home program back to primary care.

⛳️ Requirements

• Valid medical license (MD/DO).

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).

• Clinical experience in managing children with complex medical needs.

• Eligibility for licensing in PA, NL, DE, MD, and FL.

• Direct clinical experience in managing acutely ill patients.

• Background in providing comprehensive hospital-based care.

• Experience in delivering home and community care.

• Familiarity with Mobile Integrated Health (MIH) and Community Paramedicine.

• High proficiency in using Electronic Medical Records (EMRs) and remote patient monitoring tools.

• Capability to make independent clinical decisions in a non-traditional setting.

• Strong clinical judgment coupled with a systems-thinking approach.

• Exceptional verbal and written communication skills.

• Ability to thrive in a fast-paced, evolving environment with uncertainty.

• Proficient in Epic EMR, telehealth platforms, and virtual care technologies.
