This is a fully remote position, open to applicants in Canada.

📋 Description

• Facilitate and coordinate regular checkpoints with representatives from assigned Accounts.

• Maintain oversight of clinical files through collaborative interactions between clinicians and Account representatives.

• Provide efficient referral reviews in partnership with Accounts.

• Collaborate with the National Manager on complex files and requests that fall outside standard service delivery pathways.

• Monitor Key Performance Indicators by communicating with the Business Services team and utilizing internal escalation methods.

• Uphold and implement rehabilitation care models, reviewing their application by clinical teams.

• Ensure that programs and models are aligned with referral goals, generally emphasizing a strong vocational focus.

• Guarantee that access to services, assessments, treatment options, documentation, reporting, communication, and case management meet or surpass established standards.

• Evaluate scheduling practices and service delivery to ensure consistency in achieving anticipated clinical outcomes.

• Collaborate to address execution gaps and uncover opportunities for team and program/service enhancements.

• Work alongside other Account Managers to innovate service delivery and attain robust clinical outcomes, stakeholder experiences, duration of care, and sustainable results.


⛳️ Requirements

• A bachelor's degree in a relevant field.

• A minimum of 3 years of experience in a clinical or similar disability coordination position (e.g., Case Manager, Rehabilitation Coordinator).

• Required experience in the health, rehabilitation, or vocational sectors.

• Experience in the disability insurance sector, whether in an insurer or employer capacity.

• Mental health experience is considered an asset.

• Ability to provide timely, professional, and responsive service to clients.

• Comfortable facilitating and negotiating care planning.

• Must reside within Canada.
