Access Counselor, IL

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Facilitate access to care for clients, ensuring prompt and effective solutions.

• Act as the main point of contact for professional referral sources and clients during the intake process.

• Evaluate client needs, assess eligibility, and coordinate necessary services.

• Work alongside clinical and administrative teams to tackle client challenges and accelerate solutions.

• Keep meticulous records while adhering to regulatory and organizational standards.

• Establish and nurture relationships with community organizations, healthcare providers, local agencies, and other referral sources.

• Partner with business development to identify and pursue opportunities for growth.

• Represent Rosecrance at community events, networking functions, and professional gatherings.

• Interact daily with internal and external stakeholders to synchronize access and growth strategies.

• Engage in cross-functional team meetings.

• Dedicate at least one day per week to community engagement, meeting referral sources, and attending relevant events.

• Serve as a liaison between Rosecrance and the community.

• Adhere to the Rosecrance Corporate Compliance Program, Code of Conduct, mission, vision, values, and expectations.

• Provide outstanding customer service and exemplify a positive role model.

• Undertake other related responsibilities as assigned by leadership.

⛳️ Requirements

• Bachelor’s Degree in Psychology, Social Work, Counseling, or a related behavioral health field.

• Illinois Certified Addiction Counselor (CADC) or licensed in Illinois as a LSW or LPC.

• Must meet the qualifications of a Mental Health Professional (MHP) as per Illinois Department of Mental Health Rule 132.

• Demonstrated experience in behavioral health, social services, or a related field.

• Background in coordinating care with hospital systems, Employee Assistance Programs, or other safety-sensitive populations.

• Strong problem-solving skills and organizational abilities to manage complex cases and facilitate care coordination.

• Exceptional communication and interpersonal skills, both written and verbal.

• Proficient in presenting and coordinating information for clients, staff, and community sources.

• Strong computer proficiency, including the ability to operate various software applications and navigate digital platforms.

• Familiarity with the Chicago and Rockford regions is preferred.

• Valid driver’s license and ability to meet agency insurance requirements.

• Availability for occasional evening and weekend work.

• Must be an Illinois resident.

• Must successfully complete an occupational health screening.

• Must operate in a zero-tolerance drug-free environment, including marijuana.
