Youth Nurse Practitioner

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct health assessments to evaluate substance use, co-occurring conditions, and overall functioning.

• Diagnose health issues and develop tailored treatment plans.

• Prescribe and oversee medications, monitor side effects, and modify treatment as necessary.

• Collaborate with the Holon care team and external providers to ensure coordinated, holistic care.

• Act as a clinical consultant for complex or high-need cases.

• Educate clients and their families on treatment plans, medications, and strategies for recovery support.

• Maintain accurate and timely documentation in accordance with ethical, legal, and medical standards.

• Engage in team meetings, pursue professional development, and undergo continuous training.

• Provide telehealth assessments, medication management, and care coordination.

• Work closely with hybrid Care Navigator, Case Manager, and Peer Recovery Specialist teams.

• Support youth clients aged 12-17 on their recovery journeys.

⛳️ Requirements

• Active and unrestricted NP license in Tennessee.

• Active DEA license (and CDS where applicable).

• Several years of experience in behavioral health, addiction medicine, or psychiatry.

• Experience working with youth populations and their caregivers, particularly those aged 12-17.

• Proficient in addiction medicine or psychiatry.

• Board certification in specialty preferred.

• Strong documentation skills and expertise in EHR platforms.

• Clear and confident communication in virtual settings.

• Ability to work collaboratively within integrated and team-based care models.

• Organized, adaptable, and capable of navigating complex systems.

• Flexible and responsive to changing schedule needs.

• Commitment to providing person-centered care.

• Must support clients aged 12-17 facing behavioral health and/or substance use disorder needs.

• Tennessee licensure required; additional state licenses are advantageous.
