Weekend Virtual Care Nurse

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct virtual consultations and clinical evaluations with patients

• Carry out triage procedures and identify early warning signs or emerging risk patterns

• Assist in the management of chronic illnesses and provide patient education

• Track telehealth data and appropriately escalate any concerns

• Contribute to the prevention of avoidable emergency room visits and readmissions

• Work collaboratively with interdisciplinary care teams

• Accurately and promptly document patient interactions

• Engage in care planning and initiatives to enhance performance

• Aid in the establishment of best practices for virtual care

⛳️ Requirements

• Valid and unrestricted Florida LPN license

• At least 2 years of experience in home health care

• Background in triage and preventing hospitalizations

• Excellent clinical judgment and communication abilities

• Proficient in using telehealth platforms and electronic medical record (EMR) systems

• Reliable internet connection and a private workspace for remote work

• Experience with high-risk or medically complex patients is preferred

• Familiarity with chronic disease management is preferred

• Proficiency in Microsoft Office applications is preferred

• Employment is contingent on the successful completion of an AHCA Level 2 background check

• Candidates must live within Florida due to operational, licensing, and business needs
