Virtual Nurse Practitioner, Bilingual Spanish

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform initial telehealth consultations, including E/M or Annual Wellness Visits, to determine the medical necessity for care navigation services.

• Recognize high-risk conditions and address unmet social needs.

• Accurately document findings in Baba’s platform utilizing proper coding, SDOH Z-codes, diagnoses, and risk factors.

• Create and approve personalized care plans that align with patients’ medical treatment objectives.

• Work in collaboration with care advocates and nurse care managers to finalize care plans.

• Ensure that patients are enrolled into the appropriate level of support in a safe, compliant, and compassionate manner.

⛳️ Requirements

• Graduation from an accredited Nurse Practitioner program (Required).

• Active Nurse Practitioner or APRN license in good standing, with licensure in multiple states (Required).

• A minimum of 2 years of experience in primary care, telehealth, or care management (Required).

• Knowledge of Medicare programs, including care navigation, incident-to requirements, and SDOH documentation.

• Proficient in using EMRs, care management platforms, and telehealth workflows.

• Bilingual in Spanish is required.

• Legal authorization to work in the United States.

• Multiple active state Nurse Practitioner licenses.

• No disqualifying license restrictions, disciplinary actions, loss of privileges, DEA actions, or reportable malpractice payments.
