Virtual Nurse Practitioner, Bilingual Mandarin

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct initial consultations through telehealth-based Evaluation and Management or Annual Wellness Visits (AWVs).

• Establish the medical necessity for care navigation services.

• Identify high-risk conditions and unmet social needs.

• Record findings in Baba’s platform.

• Ensure accurate coding and usage of SDOH Z-codes, diagnoses, and risk factors.

• Create and approve personalized care plans.

• Validate care plans to ensure alignment with patients’ medical treatment plans and ongoing medical necessity.

• Collaborate with care advocates, nurse care managers, and the clinical operations team.

• Safely, compliantly, and compassionately enroll patients into the appropriate level of support.

⛳️ Requirements

• Completion of an accredited Nurse Practitioner program (mandatory).

• Active Nurse Practitioner or APRN license in good standing, with licensure in multiple states (mandatory).

• Minimum of 2 years of experience in primary care, telehealth, or care management (mandatory).

• Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation.

• Proficient in EMRs, care management platforms, and telehealth workflows.

• Bilingual in Mandarin is required.

• Legal authorization to work in the United States.
