Virtual Family Practice Nurse Practitioner – Temporary, NC Licensed

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Conduct virtual wellness visits for adults and/or pediatric patients, including health assessments and related healthcare services.

• Review the daily caseload and ensure technology is prepared for patient services.

• Utilize modern technology to manage the daily schedule efficiently.

• Collaborate with clinical in-home team members who visit patients at their residences.

• Establish welcoming connections with a diverse range of patients.

• Gather pertinent patient information and perform various healthcare services.

• Carry out health risk assessments, medication reviews, and cognitive screenings.

• Educate patients on their health and well-being through informative resources.

• Work alongside Sprinters to assess vital signs and facilitate blood draw arrangements.

• Execute tasks tailored to meet individual patient needs.

• Provide comprehensive care coordination and management services.

• Implement preventive care interventions, manage medications, refer to specialists, connect with community resources, and document findings.

⛳️ Requirements

• Must be board certified as a Family Nurse Practitioner.

• Hold an active Family Nurse Practitioner License in North Carolina.

• Previous experience in conducting wellness visits is required.

• Demonstrate high levels of professionalism, integrity, accountability, confidentiality, care, and compassion.

• Must be willing to work in an innovative environment that demands quick problem-solving and creative thinking.

• Technologically adept and comfortable utilizing laptops or mobile devices for charting purposes.

• Proficient in using HIPAA-secure messaging applications for effective care coordination.

• Strong written and verbal communication skills are essential.

• Ability to work independently as well as collaboratively within a team setting.

• Successfully pass a national background check and valid clinical license verification.

• Required to provide state licensure information and respond to inquiries regarding Medicare/Medicaid Annual Wellness Visits, virtual Family Nurse Practitioner experience, notice period, and prescriptive authority.
