Virtual Family Practice Nurse Practitioner – Temporary, LA Licensed

This is a fully remote position, open to applicants in Louisiana, +2 more states.

📋 Description

• Assess the daily caseload and prepare technology for patient services.

• Utilize modern technology to manage the daily schedule effectively.

• Collaborate with clinical in-home team members who visit patients' residences.

• Engage with a diverse patient population and gather pertinent information.

• Conduct virtual wellness visits, health risk assessments, medication reviews, cognitive screenings, and patient education sessions.

• Evaluate vital signs using Sprinters and arrange for blood draws as needed.

• Provide care coordination and management, encompassing preventive care, medication management, specialist referrals, community resources, and documentation.

• Offer personalized, high-quality healthcare services to patients in their homes.

⛳️ Requirements

• Must be Board Certified as a Family Nurse Practitioner.

• Hold an active Family Nurse Practitioner License in Texas, Louisiana, or Oklahoma.

• Previous experience conducting wellness visits is required.

• Demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion.

• Must be willing to engage in last-minute problem solving and think creatively.

• Proficiency in using laptops, mobile devices, and HIPAA-secure messaging applications is essential.

• Strong written and verbal communication skills are necessary.

• Ability to work independently as well as collaboratively in a team environment.

• Successfully pass a national background check and a valid clinical license search.

• Capable of conducting virtual adult and/or pediatric wellness visits.

• Relevant experience with Medicare and/or Medicaid Annual Wellness Visits is requested in the application.

• Virtual Family Nurse Practitioner experience is requested in the application.

• Current prescriptive authority in the state(s) of licensure is required, as indicated in the application.
