Virtual Family Practice Nurse Practitioner – Temporary, CA Licensed

This is a fully remote position, open to applicants in California.

📋 Description

• Review the caseload and set up technology at the beginning of the day

• Utilize modern technology to navigate the daily schedule

• Collaborate with clinical in-home team members during patient visits

• Engage with a diverse range of patients and gather pertinent information

• Conduct virtual wellness visits and health assessments for adults and/or children

• Execute health risk assessments, medication reviews, and cognitive screenings

• Provide patients with education on health and well-being

• Work alongside Sprinters to assess vital signs, coordinate blood draws, and meet patient needs

• Manage care coordination, including preventive care interventions, medication management, specialist referrals, community resources, and documentation

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner

• Active Family Nurse Practitioner License in California

• Credentialed Medicaid provider for the state of California (Medi-Cal)

• Previous experience in conducting wellness visits

• Display professionalism, integrity, accountability, confidentiality, care, and compassion

• Willingness to engage in last-minute problem solving and innovative thinking

• Proficient with technology and comfortable using laptops or mobile devices for charting and HIPAA-compliant messaging applications

• Strong communication skills, both written and verbal

• Ability to work autonomously or as part of a team

• Successfully pass a national background check and valid clinical license verification

• Experience with Medicare and/or Medicaid Annual Wellness Visits

• Virtual experience as a Family Nurse Practitioner

• Current prescriptive authority in the states where licensed to practice as a Nurse Practitioner
