Virtual Family Nurse Practitioner – NH Licensed

This is a fully remote position, open to applicants in New Hampshire.

📋 Description

• Provide patient care through telehealth consultations, encompassing telephonic triage calls, video appointments, and email follow-ups.

• Engage in ongoing education during weekly Clinical Rounds and through other learning opportunities.

• Work collaboratively with virtual and in-office colleagues during daily huddles.

• Apply specialized clinical training to deliver outstanding virtual patient care.

⛳️ Requirements

• Completed an accredited Family Nurse Practitioner (FNP) program and hold a national certification.

• In the last five years, have at least two years of experience as an Advanced Practitioner in an outpatient primary care environment treating patients of all ages (0+), OR two years in an urgent care or emergency medicine setting also treating patients of all ages (0+).

• Currently licensed in New Hampshire with the capability to acquire additional state licenses.

• Availability to work shifts on weekdays and weekends (required to work every other Saturday and/or Sunday).

• Current shift hours: 12 PM to 11 PM EST.

• Exceptional clinical and communication abilities.

• Strong commitment to human-centered primary care.

• Proficient in effectively communicating with and providing care to individuals from diverse backgrounds.

• Competence in using technology to deliver high-quality care.

• Clinical expertise in evidence-based primary care practices.

• Enthusiasm for being a vital member of a team dedicated to transforming healthcare delivery.

• Willingness to receive feedback and reflect on experiences to gain constructive insights into strengths and areas for improvement.
