Virtual Family Nurse Practitioner

This is a fully remote position, open to applicants in Illinois, +3 more states.

📋 Description

• Assess the daily caseload and set up technology for patient services

• Navigate the daily agenda utilizing technology and collaborate with clinical in-home team members visiting patients’ residences

• Engage with a diverse patient population and gather pertinent information

• Conduct virtual wellness visits and health evaluations for adults and/or children

• Execute health risk assessments, medication evaluations, and cognitive screenings

• Offer patients educational resources regarding their health and wellness

• Monitor vital signs using Sprinters and organize blood draw appointments

• Coordinate and oversee care, including preventive measures, medication management, specialist referrals, community resources, and documentation

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner

• Hold an active Family Nurse Practitioner license in KY, MI, OH, or IL

• Previous experience conducting wellness visits

• High levels of professionalism, integrity, accountability, confidentiality, care, and compassion

• Willingness to tackle challenges in a rapidly changing environment

• Proficient with technology, including laptops, mobile devices, charting tools, and HIPAA-compliant messaging applications

• Excellent written and verbal communication abilities

• Capacity to work independently or collaboratively within a team

• Must pass a national background check and valid clinical license verification

• Required to provide current state licensure details, prior experience with Medicare and/or Medicaid Annual Wellness Visits, virtual Family Nurse Practitioner experience, notice period, prescriptive authority status, and board certification status
