Virtual Clinician – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Conduct approximately 30-minute online Video Visits with members of Medicare Advantage, Medicaid, and select commercial plans.

• Review patients' medications and medical histories.

• Educate members regarding their health and respond to their inquiries.

• Assist in closing gaps in care and connect members to the appropriate healthcare services.

• Operate in a private environment with a reliable internet connection.

• Receive training and credentialing for all health plan clients and lines of business in the primary coverage state(s).

• Participate regularly in meetings with management, team members, and clinical staff.

• Achieve operational, clinical, and member experience KPIs and metrics set by Signify Health.

• Provide 40 hours of availability each week.

• Maintain a capacity of 320+ units and complete 220+ visits each month.

• Conduct Video Visits exclusively through telehealth services.

⛳️ Requirements

• Full-time, board-certified nurse practitioner or physician assistant is required.

• Current state licenses in the relevant coverage areas.

• FNP or AGNP certification is preferred for nurse practitioners.

• Must maintain board certification.

• Ability to obtain and keep multi-state licenses in the coverage area as necessary.

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth delivery, or similar formats is preferred.

• Availability for 40 hours each week.

• Consistently deliver 320+ capacity units monthly.

• Achieve a consistent pace of 220+ completed visits each month.

• Maintain training, affiliation, and compliance with Video Visit standards.

• Fulfill minimum requirements for remote visit delivery, including broadband connectivity with video and audio capabilities.

• Ensure a quiet environment with a neutral background for conducting visits.

• Ability to uphold member privacy in accordance with CVS Health guidelines.

• Strong verbal, written, and electronic communication skills.

• Capability to work independently and practice autonomously.

• Proficiency in information management and technology.
