Virtual Clinician, Home Evaluations, Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in Nebraska.

📋 Description

• Conduct approximately 30-minute online Video Visits with members enrolled in Medicare Advantage, Medicaid, and select commercial plans.

• Review patients' medications and medical history during these Video Visits.

• Educate members regarding their health and address any questions they may have.

• Assist in closing care gaps and connect members with the appropriate healthcare services.

• Offer 40 hours of availability each week, consistently delivering 320+ capacity units monthly.

• Maintain a strong visit completion rate, achieving a minimum of 220+ completed visits per month.

• Adhere to training, affiliation, and compliance standards related to Video Visit requirements.

• Engage in regular meetings with managers, team members, and clinical staff.

• Achieve operational, clinical, and member experience KPIs and metrics set by Signify Health.

• Provide telehealth services exclusively in a private environment with reliable internet access.

⛳️ Requirements

• Must be a board-certified nurse practitioner or physician assistant.

• Hold active state licenses in the respective coverage areas.

• Maintain board certification throughout your practice.

• Ability to obtain and maintain multi-state licenses in coverage areas as necessary.

• FNP or AGNP certification is preferred for nurse practitioners.

• Previous experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth service delivery, or similar formats is preferred.

• Availability of 40 hours per week is required.

• Consistent delivery of 320+ capacity units each month is expected.

• Achieve 220+ completed visits monthly.

• Comply with training, affiliation, and Video Visit requirement standards.

• Must be trained and credentialed for all health plan clients and lines of business within primary coverage state(s).

• Must meet remote visit delivery standards, including broadband connectivity with video and audio capabilities.

• A quiet setting with a neutral background is necessary for conducting visits.

• Ability to maintain health plan member privacy according to CVS Health guidelines.

• Strong verbal, written, and electronic communication skills are essential.

• Capability to work independently and practice autonomously.

• Proficiency in information management and technology is required.

• Must be able to operate a computer, phone, iPad, and other electronic devices.

• Ability to sit, write, speak clearly, and hear member responses is necessary.
