Virtual Care Nurses – RN, LPN/LVN

This is a fully remote position, open to applicants in United States.

📋 Description

• Develop comprehensive charts for Annual Wellness Visits to enhance provider efficiency and ensure compliance with CMS.

• Facilitate face-to-face virtual consultations via Zoom to discuss medical, surgical, and family histories.

• Assess medication lists and document adherence levels.

• Examine preventive screenings and immunization records.

• Conduct assessments for cognitive function, daily living activities, and depression.

• Identify overdue screenings, risk factors, and gaps in care prior to patient visits.

• Deliver concise patient summaries and highlight key findings to providers ahead of each AWV.

• Collaborate with patients to gather any missing or clarified information.

• Ensure documentation aligns with Medicare, CMS, and organizational standards.

• Maintain accurate, timely, and confidential patient records.

• Utilize CT360, RXNT, Zoom, EHR systems, and telehealth platforms effectively.

• Measure and promote outcomes related to clinical quality, patient engagement, and satisfaction.

• Comply with HIPAA, CMS, and organizational compliance regulations.

• Engage in professional development and continuous training.

• Exhibit independent decision-making, prioritization, and multitasking abilities.

• Execute additional duties as assigned.

⛳️ Requirements

• Active, unrestricted RN or LPN/LVN license in a compact state.

• Over 3 years of clinical nursing experience (bedside, Primary Care, Med/Surg, or Home Health).

• Demonstrated experience providing high-volume patient care in a dynamic virtual or telehealth clinic setting.

• Proficient in EHR systems and telehealth/teleconferencing platforms.

• A dedicated, private home workspace suitable for confidential patient interactions.

• Strong communication skills and proven ability to work independently in a remote setting.

• Employees must conduct all work physically within the United States; international work is prohibited during scheduled work hours.

• Compatible with Windows or Apple (macOS) computers only; no Chromebooks, Linux machines, or smartphones allowed.

• Windows 10 or macOS 14 Sonoma or later required.

• Intel Core i5 or AMD equivalent for Windows, or Apple Silicon for Macs.

• Minimum of 16 GB RAM.

• Wired headphones are required; built-in microphones are not acceptable.

• Minimum internet speed of 100 Mbps download and 50 Mbps upload, with an Ethernet cable directly connecting the computer to the router.

• Google Chrome must be used with Amazon Workspaces.

• A webcam is essential for video calls.

• A second monitor is highly recommended for laptop users; dual monitors are suggested for PC users.

• Preferred: Experience with Medicare Annual Wellness Visits (AWVs).

• Preferred: Familiarity with CMS documentation and preventive care guidelines.

• Preferred: Experience working across various virtual care programs and platforms.
