Virtual Care Nurse, RN, LPN/LVN

This is a fully remote position, open to applicants in United States.

📋 Description

• Create detailed Annual Wellness Visit charts to enhance provider efficiency and ensure compliance with CMS standards.

• Evaluate and summarize patients' medical, surgical, and family histories during face-to-face virtual consultations via Zoom.

• Assess medication lists and document patient adherence.

• Review preventive screenings and immunizations.

• Conduct assessments for cognitive function, functionality, and depression.

• Identify overdue screenings, risk factors, and care gaps prior to patient visits.

• Provide concise patient summaries and critical findings to providers before each AWV.

• Collaborate with patients to gather any missing or clarified information.

• Ensure documentation aligns with Medicare, CMS, and organizational standards.

• Maintain accurate, timely, and confidential patient records.

• Utilize CT360, RXNT, Zoom, electronic health record systems, and telehealth platforms.

• Measure and promote outcomes related to clinical quality, patient engagement, and satisfaction.

• Comply with HIPAA, CMS, and organizational regulations.

• Engage in professional development and continuous training.

• Make independent decisions, prioritize tasks, multitask, and carry out other assigned duties.

• Work 40 hours weekly, Monday to Friday from 9:30 AM to 6:00 PM EST, with one Saturday per month from 10:00 AM to 4:00 PM EST, including rotating holidays.

⛳️ Requirements

• Valid, unrestricted RN or LPN/LVN license in a compact state.

• Minimum of 3 years of clinical nursing experience (bedside, Primary Care, Med/Surg, or Home Health).

• Demonstrated experience providing high-volume patient care in a dynamic virtual or telehealth clinic setting.

• Proficient in EHR systems and telehealth/teleconferencing tools.

• A dedicated, private home workspace that allows for confidential patient interactions.

• Excellent communication skills and a proven ability to work autonomously in a remote setting.

• Employees must perform all work physically within the United States; international work is not allowed during scheduled hours.

• Computer must be either Windows or Apple (macOS); Chromebooks, Linux machines, or smartphones are not acceptable.

• System requirements include at least Windows 10 or macOS 14 Sonoma.

• Intel Core i5 or AMD equivalent (for Windows), or Apple Silicon (for Macs).

• Minimum of 16 GB RAM.

• Wired headphones are mandatory; built-in microphones are not permitted.

• Internet speed must be at least 100 Mbps download and 50 Mbps upload.

• An Ethernet cable should connect the computer directly to the router.

• Google Chrome with Amazon Workspaces is required.

• A webcam is necessary for video calls.

• Experience with Medicare Annual Wellness Visits (AWVs) is preferred.

• Familiarity with CMS documentation and preventive care guidelines is preferred.

• Experience in working across various virtual care programs and platforms is preferred.
