Virtual Care Nurse Practitioner – PRN

This is a fully remote position, open to applicants in Connecticut, +4 more states.

📋 Description

• Provide comprehensive, evidence-based care remotely via a technology platform.

• Evaluate, diagnose, treat, and oversee health concerns.

• Offer health counseling and care planning for patients aged 18 months and older.

• Educate and provide treatment regarding pregnancy prevention, STI prevention, and safe sex practices.

• Assess patients in primary care, acute care, chronic illness, and transitional care.

• Deliver counseling on contraceptive care and manage related medications.

• Inform patients about health maintenance and address their care inquiries.

• Record patient care in an EHR following MinuteClinic policies and protocols.

• Coordinate patient care with both internal and external colleagues, as well as the wider patient-centered medical home.

• Collaborate with Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other members of the healthcare team.

• Operate independently, prioritize tasks, resolve issues, take initiative, and advocate for patients and the practice.

⛳️ Requirements

• Valid license in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV.

• At least two years of relevant medical experience or equivalent qualifications.

• Completion of a Master’s Degree level Family Nurse Practitioner program.

• Current National Board Certification.

• State license to practice in the Advanced Practice Nurse role.

• Strong verbal, written, and electronic communication skills.

• Excellent organizational abilities and capacity to handle multiple tasks.

• Initiative, problem-solving skills, adaptability, and flexibility.

• Ability to work independently and practice autonomously.

• Proficient with information management and technology.

• Capability to collaborate effectively with professional colleagues.

• Depending on the market, fluency in additional languages, including Spanish, may be necessary.

• Reliable broadband connectivity and a quiet environment with a neutral background for remote care delivery.

• Commitment to maintaining patient privacy in accordance with CVSH guidelines.

• Capability to view and read a computer screen and other electronic devices.
