This is a fully remote position, open to applicants in Massachusetts.
• Deliver direct clinical services to patients experiencing neurological disorders, memory issues, and/or dementia through telehealth.
• Perform patient evaluations, create care plans, and oversee ongoing treatment in conjunction with supervising physicians.
• Collaborate with neurologists and the wider healthcare team to guarantee continuity of patient care.
• Inform patients and their families about diagnoses, treatment options, and the progression of diseases.
• Ensure precise and timely clinical documentation in the EMR that aligns with regulatory standards.
• Must reside in Massachusetts and hold a valid license in the state.
• Possess an active, unrestricted Nurse Practitioner license in Massachusetts.
• Preferred certification in geriatrics, adult-gerontology, or family practice.
• At least 2 years of clinical experience in neurology, memory care, and/or dementia care environments.
• Strong clinical judgment and documentation skills are essential.
• Ability to manage a full patient panel independently while collaborating with physicians.
• Exceptional communication skills when interacting with patients experiencing cognitive decline and their caregivers.
