Triage Clinical Guide, Registered Nurse

This is a fully remote position, open to applicants in Arizona, +3 more states.

📋 Description

• Offer telephonic consultation and clinical triage to members of Devoted Health.

• Interact with members through phone calls and the patient portal to deliver clinical advice, provide educational resources, address inquiries, and guide patients to the suitable level of care.

• Direct members to Devoted Medical providers, primary care, urgent care, or emergency services.

• Provide telehealth assistance for patients experiencing urgent issues across various regions.

• Oversee clinical chat queues and reply to incoming messages in the patient portal within set response timeframes.

• Elevate new active symptoms to acute clinical triage via telephone communication.

• Establish relevant Care OnDemand support episodes.

• Dispatch templated responses for non-urgent administrative or routine requests.

• Securely distribute educational materials and clinical summaries after acquiring patient consent.

• Collaborate with RNs, NPs, PAs, MDs, and medical assistants.

• Engage in learning sessions, enhance skills, and educate others.

• Advocate for patients and promote the family-oriented care model of Devoted Health.

⛳️ Requirements

• An active, unencumbered Compact RN license OR a commitment to obtain necessary additional state licenses (for non-compact states).

• At least 5 years of direct patient care experience.

• Previous clinical triage experience in an Emergency Department, Intensive Care Unit, Primary Care, or Telephonic Triage environment.

• Required prior telehealth experience.

• Willingness to learn and ability to swiftly master electronic medical records (EMR), remote telephone software, and virtual video systems.

• Outstanding communication and active listening abilities.

• Capability to work in a fast-paced environment, switch tasks efficiently, and support fellow colleagues.

• Experience in caring for older adult populations is preferred.

• Background in population health or value-based care programs is desirable.

• A BSN (Bachelor of Science in Nursing) degree is preferred.
