Travel HRA Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Travel to and from the residences of patients throughout designated regions.

• Conduct thorough, non-invasive physical assessments in patients' homes, including settings such as Long Term Care, Assisted Living, and Independent Living.

• Complete an average of 20–30 health risk assessments weekly.

• Review medical histories and perform physical examinations.

• Ensure assessments are documented accurately and in a timely manner.

• Conduct medication reconciliation to promote patient safety and adherence.

• Provide fundamental patient education regarding wellness and preventive healthcare practices.

• Collaborate with Curana and facility teams to maintain continuity of care.

• Uphold confidentiality and comply with all relevant regulatory standards.

• Travel full-time within a specified region, with possible assignments in other areas based on organizational needs.

⛳️ Requirements

• Active Nurse Practitioner Licensure in two or more states where Curana operates.

• Unrestricted licensure in (insert markets) with the capability to obtain additional licenses within the first 30 days.

• Convenient access to a major airport for frequent travel.

• Extensive experience with ICD-10 and HCC documentation.

• Willingness to travel extensively and adapt to different home environments.

• Exceptional communication and interpersonal skills.

• Proficiency in electronic medical record (EMR) systems.

• Previous experience with I-SNP or HRA vendors is preferred.

• The company is currently unable to provide visa sponsorship (H1B or otherwise).
