Contract Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Assist Sana members in maneuvering through the healthcare system to obtain suitable, high-quality, and cost-effective care.

• Aid members in locating providers, comprehending referral options, and coordinating access to healthcare services.

• Engage with members, healthcare providers, and external partners to collect information, address routine issues, and facilitate care.

• Work in collaboration with Care Navigators, Sana Care providers, Medical Assistants, and Customer Support to ensure a smooth member experience.

• Keep accurate and timely records of member interactions and care navigation activities.

• Adhere to established workflows, manage tasks to completion, and escalate complex clinical, benefits, or operational inquiries.

• Recognize opportunities for workflow enhancements and provide feedback to the team.

• Ensure members feel heard, supported, and empowered throughout their healthcare journey.

⛳️ Requirements

• Minimum of 2 years of experience in a dynamic environment.

• At least 1 year of direct experience in care navigation, referrals, provider scheduling, or healthcare benefits.

• Strong written and verbal communication abilities.

• Exceptional organizational skills and meticulous attention to detail.

• Capacity to manage several tasks and follow through consistently.

• Proficient in learning new technologies and navigating various systems.

• Ability to thrive in a fast-paced, remote setting.

• Strong problem-solving capabilities and sound judgment regarding when to seek assistance or escalate an issue.

• Collaborative mindset and receptiveness to feedback.

• Adaptability and readiness to undertake new responsibilities as team needs evolve.
