Travel HRA Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Travel to and from patients' residences within designated regions.

• Conduct thorough, non-invasive physical assessments in long-term care, assisted living, and independent living environments.

• Complete an average of 20–30 assessments weekly.

• Review medical histories and perform physical examinations.

• Ensure precise and timely documentation of assessments.

• Carry out medication reconciliation.

• Provide fundamental patient education on wellness and preventive healthcare practices.

• Collaborate with Curana and facility teams to guarantee continuity of care.

• Uphold confidentiality and adhere to regulatory standards.

• Travel full-time within a region, with possible assignments in other areas depending on organizational needs.

⛳️ Requirements

• Active Nurse Practitioner Licensure in two or more states where Curana operates.

• Unrestricted licensure in (insert markets) with the capability to obtain additional licenses within the first 30 days.

• Convenient access to a major airport for frequent travel.

• Extensive experience with ICD-10 and HCC documentation.

• Willingness to travel extensively and adapt to various home settings.

• Exceptional communication and interpersonal skills.

• Proficiency in electronic medical record (EMR) systems.

• Previous experience with I-SNP or HRA vendors is preferred.

• Ability to comply with confidentiality and regulatory standards.

• Visa sponsorship is not available.
