Travel HRA Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Travel to and from the residences of patients within designated regions.

• Conduct thorough, non-invasive physical evaluations in long-term care, assisted living, and independent living facilities.

• Complete an average of 20–30 assessments each week.

• Review patient medical histories and perform physical examinations.

• Ensure precise and timely documentation of all assessments.

• Execute medication reconciliation to guarantee patient safety and compliance.

• Provide fundamental education to patients about wellness and preventive healthcare practices.

• Work collaboratively with Curana and facility teams to maintain continuity of care.

• Uphold confidentiality and adhere to regulatory standards.

• Engage in full-time travel within a region, with possible assignments in other areas based on the organization's needs.

⛳️ Requirements

• Current Nurse Practitioner Licensure in two or more states where Curana operates.

• Unrestricted licensure in (insert markets) with the capability to obtain additional licenses within the first 30 days.

• Convenient access to a major airport for frequent travel.

• Significant experience with ICD-10 and HCC documentation.

• Willingness to travel extensively and adapt to diverse home settings.

• Strong communication and interpersonal skills.

• Proficient in electronic medical record (EMR) systems.

• Previous experience as an I-SNP or HRA vendor is preferred.

• Visa sponsorship is not available (H1B or otherwise).
