πŸ“‹ Description

β€’ Identify, supervise, and manage transitions for members within the community.

β€’ Collaborate on care plans with community care coordinators.

β€’ Provide education to transition enrollees regarding services, requirements, limitations, and exclusions.

β€’ Conduct or assist with member assessments and screenings.

β€’ Develop or support the creation of member transition, service, or care plans.

β€’ Assist care coordination teams, providers, and healthcare professionals in formulating effective transition plans.

β€’ Aid in transitions based on enrollment or transition-of-care services.

β€’ Identify new enrollees who require transition services.

β€’ Ensure that existing authorizations are honored during transitions and address any issues with care management teams and providers.

β€’ Act as a resource for members, families, and caregivers.

β€’ Track and maintain transition metrics while documenting transition-of-care information.

β€’ Create educational materials and resources.

β€’ Support education and training initiatives for care coordination staff and providers.

β€’ Assess member needs and available resources, recommending and facilitating appropriate plans.

β€’ Coordinate communication between members, families/caregivers, and care provider teams.

β€’ Engage with healthcare providers to enhance care coordination.

β€’ Adhere to policies and standards while performing other assigned responsibilities.


⛳️ Requirements

β€’ A valid California LVN/LPN state license is mandatory.

β€’ A Bachelor's degree is required.

β€’ 2–4 years of relevant experience are necessary.

β€’ Equivalent experience gained through applicable knowledge, responsibilities, scope, and skills may substitute for the specified education/experience requirements.

β€’ Must be available to work PST hours.

β€’ Experience in charting, case management, discharge planning, and telephonic care coordination is preferred.
