Transition Specialist, LVN/LPN

This is a fully remote position, open to applicants in California.

📋 Description

• Identify, supervise, and manage transitions for members within the community.

• Collaborate on care plans with community care coordinators.

• Provide education to transition enrollees regarding services, requirements, limitations, and exclusions.

• Conduct or assist with member assessments and screenings.

• Develop or support the creation of member transition, service, or care plans.

• Assist care coordination teams, providers, and healthcare professionals in formulating effective transition plans.

• Aid in transitions based on enrollment or transition-of-care services.

• Identify new enrollees who require transition services.

• Ensure that existing authorizations are honored during transitions and address any issues with care management teams and providers.

• Act as a resource for members, families, and caregivers.

• Track and maintain transition metrics while documenting transition-of-care information.

• Create educational materials and resources.

• Support education and training initiatives for care coordination staff and providers.

• Assess member needs and available resources, recommending and facilitating appropriate plans.

• Coordinate communication between members, families/caregivers, and care provider teams.

• Engage with healthcare providers to enhance care coordination.

• Adhere to policies and standards while performing other assigned responsibilities.

⛳️ Requirements

• A valid California LVN/LPN state license is mandatory.

• A Bachelor's degree is required.

• 2–4 years of relevant experience are necessary.

• Equivalent experience gained through applicable knowledge, responsibilities, scope, and skills may substitute for the specified education/experience requirements.

• Must be available to work PST hours.

• Experience in charting, case management, discharge planning, and telephonic care coordination is preferred.
