This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Identify, supervise, and manage the coordination of member transitions within the community.

• Ensure that transitions between different levels of healthcare services are conducted safely and promptly.

• Collaborate with community care coordinators to coordinate care plans.

• Educate members undergoing transitions about available services, requirements, limitations, and exclusions.

• Support care coordination teams, providers, and other healthcare team members in formulating transition plans.

• Assist with transitions related to enrollment or transition-of-care services.

• Identify new member enrollees who require transition services.

• Ensure that existing authorizations are respected during transitions.

• Act as a resource for members, families, and caregivers.

• Monitor and maintain transition metrics while documenting transition-of-care information.

• Prepare educational materials and resources.

• Aid in the development of education and training programs.

• Assess member needs and available resources, recommending and facilitating appropriate care plans.

• Coordinate communication between members, caregivers, and care provider teams.

• Engage with healthcare providers to promote effective care coordination.

• Fulfill other assigned responsibilities while adhering to policies and standards.


⛳️ Requirements

• A Bachelor's degree is required.

• 2–4 years of relevant experience is preferred.

• Equivalent experience may be obtained through demonstrated knowledge, responsibilities, scope, and skills relevant to this position.

• A background in healthcare and/or behavioral health is essential.

• Knowledge of Wisconsin Medicaid is beneficial.

• Capability to manage a high volume of inbound and outbound calls.

• Proficiency in conducting assessments and creating individualized care plans.

• Availability from Monday to Friday, 8 AM to 5 PM CST.
