TeleHealth RN – Per Diem

This is a fully remote position, open to applicants in California.

📋 Description

• Manage designated inbound and outbound calls within the scope of practice, collaborating with clinicians and on-call providers.

• Organize telehealth meetings between participants and clinicians.

• Ensure prompt delivery of care and address basic issues, escalating to management when necessary.

• Address logistical care delivery challenges related to transportation, medication delivery, and hospital discharges.

• Assist clinical care and home health teams in facilitating transitions between hospitals, skilled nursing facilities, and other environments.

• Resolve logistical challenges via phone and during visits to participants’ homes or acute care facilities.

• Consult with other clinical staff on call as required.

• Foster ongoing relationships with participants.

⛳️ Requirements

• Graduate from an Accredited School of Nursing with a valid, unrestricted RN license.

• Possess nursing knowledge and skills essential for treating frail, elderly participants and managing complex clinical scenarios.

• Highly driven and self-motivated.

• Capable of performing tasks in a rapidly changing environment.

• Able to make informed decisions in emergency situations.

• Excellent clinical, organizational, and communication skills, particularly with seniors, their families, and interdisciplinary team members.

• Available to work assigned shifts, including days, evenings, nights, weekends, holidays, and overtime as needed.

• Experience in triaging is preferred.

• Proof of vaccination or a vaccine declination form is required prior to employment.

• Must comply with infection control, PPE, and vaccination requirements.
