Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in United States.

📋 Description

• Primarily conduct telehealth video consultations, with limited and occasional in-home visits to local members.

• Carry out thorough assessments and deliver evidence-based care emphasizing prevention, chronic condition management, medication optimization, and health maintenance.

• Recognize and address gaps in care.

• Collaborate with both internal and external care team members.

• Contribute to personalized, whole-person care strategies.

• Leverage integrated technology and AI-driven workflows to minimize administrative tasks and enhance meaningful interactions with members.

• Communicate with empathy and clarity, building trust and confidence with members and their caregivers.

• Work closely with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to provide coordinated, member-centered care.

• Establish relationships with members while operating in a supportive, tech-enabled environment.

⛳️ Requirements

• Current Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• Over 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.

• Valid and active NP/PA license.

• Willingness to obtain additional licenses, if required, within the first 90 days; support and reimbursement will be provided.

• Active BLS certification at the time of hire.

• Comfort with providing care via video telehealth and navigating electronic documentation systems.

• Knowledge of managed care models, including STARS/HEDIS and identifying care gaps.

• Experience conducting comprehensive or preventive care visits with Medicare populations.

• Experience assisting older adults and/or individuals with complex medical or social needs.
