Telehealth Physician, FT

This is a fully remote position, open to applicants in Illinois, +1 more state.

📋 Description

• Conduct regular telehealth consultations with Medicare patients, encompassing E/M and Chronic Care Management.

• Develop and oversee tailored care plans for ongoing health conditions.

• Work in collaboration with the integrated care team, which includes nurse case managers, nurse practitioners, and community health workers.

• Maintain precise documentation in the electronic health record (EHR) system and ensure adherence to quality standards.

• Motivate patients to engage in preventive care practices to minimize the risk of avoidable complications.

⛳️ Requirements

• MD or DO degree.

• Board Certified or Eligible in Family Medicine, Internal Medicine, or Geriatrics.

• Valid medical license in Georgia.

• Multi-state licensure is an advantage.

• Credentialed with major insurance providers: UHC, Aetna, Humana.

• Strong passion for preventive care, senior health, or management of chronic conditions.

• Proficient in technology and comfortable delivering care through telehealth platforms.

• Collaborative, mission-oriented attitude with outstanding communication skills.
