Telehealth Physician

This is a fully remote position, open to applicants in Florida, +3 more states.

📋 Description

• Conduct routine telehealth consultations with Medicare patients, encompassing E/M and Chronic Care Management.

• Develop and oversee individualized care plans for ongoing health issues.

• Collaborate with the integrated care team, which includes nurse case managers, nurse practitioners, and community health workers.

• Ensure comprehensive documentation in the electronic health record (EHR) and adhere to quality compliance standards.

• Encourage patients to engage in preventive care to minimize avoidable health complications.

⛳️ Requirements

• MD or DO degree.

• Board Certified or Eligible in Family Medicine, Internal Medicine, or Geriatrics.

• Active medical license required.

• Multi-state licensure is advantageous.

• Credentialed with major insurance providers: UHC, Aetna, Humana.

• Strong passion for preventive healthcare, senior health, or management of chronic conditions.

• Tech-savvy and proficient in delivering care via telehealth.
