Telehealth Nurse Practitioner – Per Diem Weekends

This is a fully remote position, open to applicants in Florida, +5 more states.

📋 Description

• Deliver diagnostic patient care primarily through virtual remote consultations via video conference or telephone.

• Provide on-call support and act as the dispositional authority for after-hours and weekend clinical inquiries.

• Evaluate, diagnose, treat, and document the physical and psychosocial health status of geriatric patients.

• Commit to a minimum of 72 weekend hours every three months and at least 160 hours annually.

• Be available as needed, including nights, weekends, and holidays.

• Cover shifts for PTO, leave of absence, and unanticipated absences.

• Conduct virtual evaluations for both acute and non-acute clinical issues.

• Execute and document physical assessments and patient histories.

• Analyze trends and create, document, and implement patient management strategies.

• Provide health education, physician referrals, case management referrals, and counseling for patients and families.

• Assess the need for immediate nursing intervention, consultation, or referral.

• Gather and evaluate comprehensive and focused health data for patients and families.

• Collaborate with primary care providers (PCPs) and other medical professionals regarding health data and adherence to guidelines.

• Advise patients and their families on health outcomes and encourage positive health behaviors.

• Lead initiatives to achieve optimal patient outcomes through telemedicine.

• Work with on-call PCPs to implement procedures aimed at achieving desired outcomes.

• Record assessments, interventions, and progress in a clear and translatable format.

• Improve telehealth practices and systems through analysis of program effectiveness data.

• Utilize various communication formats, tools, and technologies to provide care.

• Engage in clinical rounds, conferences, risk and quality management programs, and relevant meetings.

• Comply with departmental standards, regulatory requirements, quality-care policies, and practice agreements.

• Collaborate with physicians to develop telehealth policies, procedures, and protocols.

• Undertake additional duties as assigned by management.

⛳️ Requirements

• Bachelor’s degree in Nursing (BSN) and completion of a nursing school for Advanced Practice Nursing with specialty certification required.

• Master’s degree in Nursing is mandatory.

• A valid, active Registered Nurse license is essential.

• Board certification by AANP or ANCC required for Nurse Practitioners.

• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross must be obtained within the first 90 days of employment.

• Current DEA number for Schedule II-V controlled substances may be necessary based on the state of practice.

• Multi-state licensure required, including FL, VA, and at least two licenses among GA, KS, MI, MO, NC, OH, PA, SC, TN, TX, IL, KY, and LA.

• A minimum of 3 years’ experience in acute/primary care clinical settings is required.

• At least 2 years of telehealth experience is preferred.

• Strong business acumen and understanding of job-related functions, practices, processes, procedures, techniques, and methods.

• Knowledge of medical practices to operate independently as a certified practitioner and collaborate effectively with physicians and other providers.

• Proven track record of meeting clinical performance metrics.

• Technical proficiency to conduct telemedicine consultations in compliance with state and federal regulations.

• Exceptional clinical judgment and problem-solving skills.

• Ability to prioritize tasks and work efficiently under pressure.

• Strong communication and collaboration skills with physicians, patients, and team members.

• Capability to work effectively within a multidisciplinary team.

• Willingness to travel locally within South Florida for meetings and training sessions up to 10% of the time.

• Availability for evenings, overnights, weekends, and at least three holidays each calendar year.

• Proficient in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook.

• Fluent in spoken and written English; bilingual in Spanish/Creole is an advantage.
