Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Perform thorough health assessments through telehealth services

• Record risk adjustment (HCC coding) during patient consultations

• Address HEDIS care gaps during patient interactions

• Evaluate medical history, current medications, and preventive care needs

• Accurately document visits using ICD-10 and CPT II coding

• Provide clear care plans along with follow-up instructions

• Ensure precise visit documentation to support quality initiatives

⛳️ Requirements

• Valid Nurse Practitioner license in Tennessee

• Preference for multi-state NP licenses

• Minimum of 1 year experience in Family Medicine or Internal Medicine

• Experience working with Medicare and Medicaid populations

• Knowledge of HEDIS and risk adjustment processes

• Required enrollment as a Medicare and Medicaid provider

• Proficient in delivering care via telehealth platforms

• Ability to consistently work during weekday daytime hours, with a preference for availability on Thursdays and Fridays
