Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in California.

📋 Description

• Perform thorough health assessments through telehealth services

• Accurately document risk adjustment (HCC coding) during patient consultations

• Address HEDIS care gaps during patient appointments

• Evaluate medical history, medications, and preventive care requirements

• Record visits utilizing ICD-10 and CPT II coding systems

• Provide clear care plans along with follow-up instructions

• Ensure precise documentation of visits for quality assurance programs

⛳️ Requirements

• Valid Nurse Practitioner license in California

• Preference for multi-state NP licenses

• Over 1 year of experience in Family Medicine or Internal Medicine

• Experience with Medicare and Medicaid patient populations

• Knowledgeable in HEDIS and risk adjustment processes

• Required enrollment as a provider for Medicare and Medicaid

• Proficient in delivering care via telehealth platforms

• Willingness to work consistently during weekday daytime hours, with a preference for availability on Thursdays and Fridays
