This is a fully remote position, open to applicants in Tennessee.
• Assist in promoting the physical, behavioral, and social health and well-being of members.
• Support members in securing and retaining competitive, integrated employment.
• Manage complex relationships to ensure members access essential services.
• Conduct intake, enrollment, and continuous assessments.
• Create and uphold Person-Centered Plans.
• Oversee and ensure the quality of service delivery.
• Collaborate with members and their families to identify necessary supports and direct services aimed at health, employment, and goals related to independent or community living.
• Work alongside clinical and social support teams to address members’ physical, behavioral health, and long-term service requirements.
• Carry out comprehensive, impartial face-to-face evaluations to assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent living needs.
• Implement criteria to authorize suitable home- and community-based services.
• Verify that authorized services are delivered and that members' needs are fulfilled.
• Travel to the residences of members located in the Monroe, McMinn, and Polk County areas.
• RN with a preference for current Developmental Disabilities Nurses Association certification as CDDN for RNs or DDC for LPNs, as applicable.
• Alternatively, a B.S. in social work, nursing, education, human services, healthcare profession, or a related field.
• Or fulfillment of federal criteria for a Qualified Developmental Disabilities Professional (QDDP or QIDP).
• Or, if lacking a degree, at least 5 years of experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, along with completion of Personal Outcome Measures Introduction and Assessment Workshop trainings established by the Council on Quality and Leadership.
• Minimum of 2 years of experience within the healthcare sector or a similar operational environment is required.
• A valid driver's license is necessary.
• Various immunizations and/or related medical tests may be mandatory.
• A digital literacy assessment is required.
• Proficient use of Microsoft Office (Outlook, Word, Excel, and PowerPoint) is required.
• Capability to work independently with minimal oversight or collaboratively within a team.
• Ability to resolve problems and manage multiple tasks under tight deadlines.
• Competence in handling several projects simultaneously.
• Exceptional oral and written communication abilities.
• Strong interpersonal and organizational skills.
• Highly organized, dependable, and capable of managing numerous tasks with an outstanding work ethic.
• For routine company driving: valid driver's license, appropriate auto insurance where applicable, and an acceptable motor vehicle record.
• Mileage reimbursement for travel to members’ homes.
